How to Find Property Survey and Media Sanitization Certification

  1. Visit the Property Management Forms page, and select Media Sanitization Certificate.  Media Sanitization Certificate link within sub-categories of Property Management Forms page
  2. Login will be required using your BeachID credentials, after which the Property Media Sanitization Certification form will open.  Fill out all information (i.e. Tag Number, Serial Number, Description, Etc.)and check the appropriate boxes for the reason of the item survey.
